From the Hospital:

  1. The doctor may make a recommendation for stay at a skilled nursing facility to allow for recovery and/or rehabilitation.
  2. Hospital Staff will provide names of facilities for your consideration.
  3. The Family or Resident Representative is invited to call to schedule an appointment to learn more about the nursing home, to tour the nursing home and begin the application process ask for our Admissions Coordinator.
  4. Application and Admission Process: There will be a number of documents and forms to be reviewed, completed and signed prior to admission and the day of admission.
  5. Once you have made the decision to be admitted to Pleasant View, the hospital staff and the PV Admission Nurse will coordinate the necessary arrangements for discharge from the hospital and admission to the nursing home.

From Home:

  4. An updated History and Physical is required prior to admission.
STEP ONE

  • ·The Admission Nurse at Pleasant View Skilled Nursing & Rehab (PV) is contacted by the referral party regarding interest in admission to PV.
  • Applicant and/or Family Member will provide copies of all insurance related cards. This information is required to verify benefits and eligibility for medicare and insurance coverage while at a skilled care facility.

    • Insurance Cards
    • Social Security Card
  • The PV Admission Nurse will complete the “Referral Application” form, and the “Pre-Admission Health Information” form – contacting the hospital or the Physician Office.

 

STEP TWO

  • The PV Admission Nurse will schedule a meeting with the Resident Representative and/or applicant to review the information provided on the “Referral Application” and the “Pre-Admission Health Information” forms.
  • The PV Admission Nurse will provide a tour and discuss information about services and the payment process for services in a skilled care facility.

 

STEP THREE

  • The PV Admission Nurse will review the “Referral Application” and the “Pre-Admission Health Information” with the Admission Committee to discuss the decision to approve or disapprove the Admission Application.
  • The PV Admission Nurse will notify all parties of the decision for admission.
  • Applicant and/or Family Member will provide a copy of the applicant’s Health Care Power of Attorney, Power of Attorney for Finance, Advance Directives and Living Will.

 

STEP FOUR

  • The PV Admission Nurse will request the required medical information for admission.
    • History and Physical, Chest X-ray; free of communicable disease.
    • Discharge Summary (Hospital Only)
    • Physician Orders
  • The PV Admission Nurse will coordinate the plans for Admission
    • Arrival Time
    • Room Number
    • “What to Bring” Information
  • The PV Nursing Staff will greet and escort to room and assist with settling in.
  • A Nurse, Social Worker and Therapist will be completing an Admission Assessment which is required to be completed on the day of admission.
